Will look at RNs with LTC experience! Hospice/Home Health experience preferred.

40-hour salaried full-time positions with participation in on-call coverage as required.
$85,000-$97,000 annually.

Census throughout the state of Mass is close to 100.

8a-5p business hours. Once a month on call rotation as far as call frequency.
10-15 patients Case Managed for an RN
5-6 visits a day typically
Admission and recert visits may mean being flexible in readjusting your schedule

Key responsibilities include:
– Completing comprehensive patient and family assessments
– Developing and revising individualized plans of care
– Providing skilled nursing care and symptom management
– Coordinating care with physicians, facilities, families, and the interdisciplinary team
– Managing documentation timely and accurately within the EMR system
– Ordering medications, DME, and supplies as appropriate
– Supervising ancillary staff and coordinating visit schedules
– Participating in on-call rotation and IDG meetings


The ideal candidate will have:
– Current RN licensure
– Hospice and/or home health experience preferred
– Strong assessment and critical thinking skills
– Excellent organizational and communication abilities
– Ability to work independently while collaborating with the team
– Compassionate approach to patient-centered end-of-life care
